Puk code missing

Hallo, 

 i blocked my prepaid kpn sim, and i need Puk code , but i lost it.

I tried to request Puk code on KPN website, i put my 06 numer and simkartnumber, but it says that comination is not correct…

I am sure i oput correct 06 number and correct sim number…

is prepaid sim..

what to do? help
