Hello. My SIM card has been locked. 
How can I send the request for the activation?

Hi Yasuhiro, 

What do you mean by blocked? Have you entered your pin code wrong and is it now asking for a puk code? Or something else?

My kids entered the wrong pin code and the SIM card is asking for a PUK code.

You can find your puk by filling in this form. Let me know how it goes.
